Fatigue is a symptom, not a single diagnosis. It can sit alongside iron deficiency, thyroid dysfunction, vitamin deficiency or altered blood sugar, but it may also relate to sleep, stress, mood, infection, medicines, pain, heart or lung conditions and many other factors.

Why tiredness can require more than one kind of assessment

People use the words tiredness, fatigue, exhaustion and sleepiness in different ways. Some feel an overwhelming need to sleep. Others describe reduced physical stamina, poor concentration or a sense that ordinary activity now requires disproportionate effort. The distinction matters because the likely causes and next steps may differ.

A British Journal of General Practice study of unexplained fatigue found that broad early testing did not automatically produce a useful diagnosis, supporting a more considered approach to test selection.1 A population study in BMJ Open also highlighted the importance of sleep disturbance, depression and anaemia within the wider assessment of fatigue.2

Iron status and blood loss

Iron deficiency can be relevant when tiredness occurs with heavy periods, other blood loss, restricted intake, pregnancy related changes or previous low results. Ferritin is useful, but it is not a complete story because inflammation can affect it and a low iron result does not identify the underlying cause. UCL published professional guidance that places iron deficiency anaemia within a wider management and self care pathway rather than treating the laboratory number in isolation.3

Thyroid function

Thyroid conditions may be associated with altered energy, temperature sensitivity, bowel change, skin and hair changes, palpitations or weight change. None of those symptoms proves a thyroid disorder. Results can also be affected by medicines, illness, pregnancy and laboratory interference. A focused thyroid test therefore works best when there is a clear question and a plan for follow up.

Vitamin status and supplement use

Vitamin B12, folate and vitamin D may be relevant where diet, absorption, medicines, previous deficiency or limited sun exposure create a reason to test. More is not always better. Supplements can change results, and high dose biotin may interfere with some immunoassays, including thyroid related tests. A UK study in the Annals of Clinical Biochemistry examined this interference in routine samples.4

Longer term blood sugar

HbA1c can provide context about longer term glucose exposure. It may be relevant when fatigue sits with diabetes risk factors, thirst or frequent urination. A Diabetologia report on UK Biobank data discussed the potential for HbA1c screening to identify previously undiagnosed diabetes earlier, while also noting the limits of risk based selection.5

Fatigue after exertion and ME or CFS

A standard home panel cannot diagnose myalgic encephalomyelitis or chronic fatigue syndrome. The University of Oxford has described promising experimental work on blood cell signatures, but the university also notes that there is no single established sensitive and specific diagnostic test in routine use.6 Severe or prolonged worsening after activity deserves clinical assessment rather than repeated consumer testing alone.

When fatigue may signal something more serious

New fatigue can occasionally sit alongside serious disease, particularly when combined with weight loss, bleeding, persistent fever, significant pain or other concerning symptoms. Recent British primary care research has examined underlying disease risk among patients presenting with fatigue.7 The presence of a home test should never delay appropriate medical review.

Record medicines

List prescribed medicines, over the counter products and recent changes that may influence symptoms or results.

Record supplements

Include iron, vitamin D, B12, folate, biotin and multivitamins with the dose and last time taken.

Check fasting

Some biomarkers require specific timing or fasting. Follow the instructions supplied with the chosen product.

Note recent illness

Infection, inflammation, dehydration and recovery from illness may alter several laboratory measurements.

A focused iron, thyroid, vitamin or HbA1c test may be clearer when the question is specific. A general wellness panel may be considered when a broader but still defined baseline is appropriate.

Can a home blood test diagnose why I feel tired?

No. A home blood test can provide selected laboratory information. It cannot assess sleep, mood, physical examination findings, infection, heart or lung disease, medicine effects, neurological conditions or every other explanation for fatigue.

Which test may be relevant if I have heavy periods and fatigue?

An iron profile may be relevant because blood loss can reduce iron stores. Heavy bleeding itself also deserves appropriate assessment. A result should be interpreted alongside symptoms, menstrual history, diet and inflammation.

Can thyroid problems cause low energy?

Yes. Thyroid dysfunction can be associated with low energy, temperature sensitivity, altered weight, bowel change, skin or hair changes and mood symptoms. These features are not specific, so results need clinical context.

Can vitamin B12, folate or vitamin D affect energy?

Is HbA1c useful when tiredness occurs with thirst or frequent urination?

Do I need the largest panel available?

What if every result is within the laboratory range?
